Integrated Support Across Five Core Pillars, Delivered Through Eleven Accredited Registration Groups

We provide safe, coordinated, community‑based support for aged care consumers and NDIS participants. Our model is built around five core pillars of everyday support, strengthened by six complementary service areas that extend independence, stability, and long‑term well-being.

Community nursing is available strictly within our scope of practice and scheduling capacity, ensuring all clinical services are delivered safely and appropriately.

Our Five Core Pillars of Support

Reliable, respectful support with daily routines such as showering, dressing, grooming, mobility, and meal preparation. Supported by: Household Tasks, Assist‑Travel/Transport, Development‑Life Skills, Community Nursing Care.

Specialised support for people with complex clinical needs, delivered under strong clinical governance. Supported by: Community Nursing Care, Daily Tasks/Shared Living, Group/Centre‑Based Activities, Innovative Community Participation.

Clinical care delivered in the home or community by qualified nurses. Services include wound care, medication administration, chronic disease support, health monitoring, clinical assessments, and post‑hospital care. Important: Nursing is delivered only within our defined scope of practice and subject to clinical scheduling availability. Supported by: Assist Personal Activities, Assist Personal Activities High, Household Tasks, Participate Community.

Practical assistance that keeps the home safe, clean, and functional. Supported by: Daily Tasks/Shared Living, Development‑Life Skills, Community Nursing Care.

Support to stay connected, active, and engaged in the community. Supported by: Assist‑Travel/Transport, Innovative Community Participation, Group/Centre‑Based Activities, Assist Access/Maintain Employment, Community Nursing Care.

Structured programs that support routine, social connection, and wellbeing in a safe, supervised environment. These activities provide opportunities for learning, recreation, and community belonging.

Safe, reliable transport that enables access to appointments, community activities, work, study, and essential services. This service reduces barriers to participation and ensures continuity of care.

Support for people living in shared or supported environments to build routines, maintain their home, and live safely and comfortably. This service strengthens independence and stability in shared living settings.

Practical support to explore work or study options, build vocational skills, and maintain employment pathways. This service helps participants pursue meaningful, long‑term economic and social participation.

Training and coaching in everyday skills such as budgeting, cooking, communication, travel training, and personal organisation. This service builds capability, confidence, and greater independence across daily life.
